Hypercare Update

On Friday, July 29th, the Oxford OHT’s Palliative Care Outreach Team (PCOT) implemented Hypercare secure messaging: an application that enables secure, efficient, and timely communication between physicians and patient care teams.

Two of our partnering organizations participated in the first phase of this project, Home and Community Care Support Services (HCCSS) and CarePartners. Since the project launch, Hypercare has been consistently used by physicians, nurse practitioners, nurse managers, and care coordinators to communicate with each other to care for palliative patients and their caregivers.

We are pleased to share the following metrics which highlight the ways Hypercare is being used by these care teams, and we look forward to watching these metrics grow as we expand the project’s scope over the coming months.
