Find The Right Service

It can be difficult to find the health and social services that are available in your community. We are working towards a system that will give you 24 hours a day, 7 days a week access to health and social service support.

Are you or someone you know looking for community health services?

For those 55+ or 18+ with disabilities, we can connect you to services such as:

  • Dementia and Brain Injury Support
  • Exercise Programs
  • Foot Care
  • Homemaking Supports
  • Meals on Wheels
  • Transportation

Connects you to a central intake coordinator to discuss your needs and connect you with services in your community.

No referral is needed. Fees for some services may apply. Speak with your coordinator if you have concerns
