Alzheimer Society Southwest Partners Spotlight

Alzheimer Society Southwest Partners offer a wide range of programs and services designed for individuals with dementia and/or memory concerns, their family, caregivers, and health professionals, as well as the general public. 

When asked why AlzSWP joined the OHT, they shared they felt it was important for them to be part of the healthcare system transformation and to foster new partnerships. Since joining, the AlzSWP has cultivated new collaborations that have helped them to streamline their referral processes and to receive covid-related support and guidance. One of their most notable collaborations was the recent unification of the Alzheimer Society of Elgin, London and Middlesex and Oxford County. These groups were previously separately incorporated, but are now a single unified organization that is better positioned to service the steadily increasing population affected by dementia.
