Oxford County Community Health Centre (CHC)

The Oxford Ontario Health Team is made possible through the collaborative participation of healthcare partners in our region. Today we are introducing the Oxford County Community Health Centre – OCCHC. CHCs are non-profit, multi-disciplinary centres established and governed by community members.
The Oxford CHC employs teams of nurses, nurse practitioners, social workers, dietitians, health promoters, housing stability workers, outreach workers, physicians and more. The teams work collaboratively to provide coordinated care to priority populations, such as persons living with addictions or mental health challenges, homeless or precariously housed, families with low incomes, youth, and isolated seniors.
The Oxford CHC joined the Oxford OHT as a partner in an effort to collaboratively expand the scope of services they provide to the community and to access resources that will continue to improve their patient care.
Since joining the OHT, the CHC has collaborated with OHT partners such as Indwell and CMHA Thames Valley to support programs such as Rapid Access Addiction Medicine (RAAM; https://www.oxfordraam.ca/), Housing Stability, and Outreach. The CHC is also able to reach more community members through its Mobile Health Outreach Bus, which now stops at partner locations, such as the Ingersoll Nurse Practitioner Led Clinic, to support those who are homeless and precariously housed with hot beverages and showers.
Together with partnerships and community we can continue to make Oxford a better place.
