Hypercare Update

November, 2022

The Oxford OHT’s Palliative Care Outreach Team is pleased to announce the implementation of Hypercare Secure Messaging with Oxford County Paramedic Services (OCPS). This phase of the Hypercare Project introduced 20 new users to the platform, including OCPS superintendents, supervisors, administrators, and paramedics.

Since this implementation on October 19th, trained OCPS staff can communicate directly with physicians and multi-disciplinary care team members from across our partnering organizations, CarePartners and Home and Community Care Support Services Southwest. Hypercare allows for secure and prompt coordination when it comes to patient care, improving the efficiency of the care team and helping community members avoid unnecessary hospital visits.

According to OCPS, “Hypercare has made it easier for us to communicate reliably and securely with each patient’s care team. Having everyone on board with one technological solution is imperative to ensuring the most effective and efficient care.”

Thank you to our partners who have learned and embraced this platform so far – we look forward to continuing to build on this successful and coordinated method of communication.
